If you have participated in an earlier survey please read through the following directions:

If you have participated in a previous survey and do not recall your username and/or your password, please take your new token and register here. Please note that if you receive an "Username already exists" error you may have an account created during an earlier survey instance. If this happens simply try registering again with a differnt username.

If you have participated in a previous survey and used your email address as your username and have lost or do not recall your password from an earlier survey please click here to reset your password.

If you have participated in a previous survey and recall your login information please read through the following directions on adding a survey token to you existing SAI account.

Taking a new Survey

Start on the SAI Login Page

Enter the User Name and Password you created for an earlier survey

Adding your 2009 Survey Token

The images below are only examples and may or may not correspond exactly with what you will see once logged into the application.

Once logged in you will be on your SAI Home page

Add your 2009 Survey Token in the Add Token Field and click on the Add Token button.

Click on the newly added link to take the 2009 Survey
